ABSTRACT

Objective To investigate the effects of dexmedetomidine (DEX) on the mRNA expression of triggering receptor expressed on myeloid cells 1 (TREM-1) in peripheral blood mononuclear cells of rats with acute obstructive suppurative cholangitis (AOSC).Methods Sixty healthy male Wistar rat models of AOSC induced by complete common bile duct ligation and injection of E.coli into the bile duct through an intubation tube were replicated successfully.After modeling,the peripheral blood was collected and mononuclear cells were isolated and cultured.According to random number table method,the mononeuclear cells were divided into model group (no drug added in culture of mononuclear cells) and low,medium and high dose DEX groups (final concentrations 0.4,0.8,1.2 μg/L DEX were in low,medium and high DEX mononuclear cell cultures,respectively).After the mononuclear cells were cultured for 24 hours,the levels of tumor necrosis factor-α (TNF-α),interleukins (IL-1 and IL-6) in the supernatant of the cultured mononuclear cells were detected by enzyme linked immunosorbent assay (ELISA).The level of C-reactive protein (CRP) was detected by immunity transmission turbidimetry.The expression of TREM-1 mRNA in the mononuclear cells was detected by reverse trantscription-polymerase chain reaction (RT-PCR).Results Compared with the model group,the levels of TNF-α,IL-1,IL-6,CRP were decreased,the TREM-1 mRNA expressions were down-regulated in the different DEX dose groups,and the degrees of descent in medium and high dose groups were more significant than those in low dose group [TNF-oα (ng/L):95.5±8.6,88.9±5.3 vs.131.1 ± 14.2;IL-1 (ng/L):53.5±8.3,48.3 ± 6.7 vs.73.7 ± 12.8;IL-6 (ng/L):266.9±26.2,252.1 ± 17.7 vs.349.9±40.4;CRP (ng/L):4.3 ± 1.1,3.9 ±0.7 vs.5.6 ± 1.7;TREM-1 mRNA (A value):0.43 ± 0.18,0.39 ± 0.16 vs.0.65 ±0.25,all P < 0.05].Conclusion DEX can down-regulate the expression of TREM-1 mRNA and inhibit the formation and secretion of inflammatory factors TNF-α,IL-1,IL-6 and CRP in peripheral blood mononuclear cells of rats with ASOC.

ABSTRACT

Objective To investigate the clinical features and treatment points of diabetes patients compli-cated with parapharyngeal abscess .Methods The clinical data of 12 diabetes mellitus patients complicated with parapharyngeal abscess were analyzed .Results 12 patients were all cured and discharged without concurrent medias-tinal abscess ,intracranial infection and serious complications such as bleeding .Conclusion The development of dia-betes mellitus complicated with parapharyngeal abscess is rapid ,we must confirm the diagnosis ,timely implementation of abscess incision and drainage ,effective drainage of pus ,keep the airway open ,actively control the infection at the same time strictly control the fluctuations in blood sugar ,strengthen supportive therapy ,thus to improve treatment and reduce the incidence of complications .

ABSTRACT

Objective To analyze the genotype of varicella-zoster virus (VZV) strains isolated from patients with chickenpox or zoster and to differentiate them from Oka vaccine strain by molecular analysis. Methods In the present study, single nucleotide polymorphisms(SNP) based VZV genotypes were analyzed in 19 VZV isolates using the polymerase chain reaction and restriction fragment length polymorphisms analysis of DNA fragments of the open reading frames 38, 54, 62, and the 1t5 repeat region. Results The genotypes of 19 VZV isolates including two different groups with 52.7% of Pst Ⅰ+ Bgl Ⅰ+ R5A and 47.3% of Pst Ⅰ+ Bgl Ⅰ+ R5B, which is very different from those found in North America, Europe and Japan. All the Chinese isolates are wild-type strains with ORF62 Sma Ⅰ-. No Oka vaccine strains were revealed among the isolates. Conclusion Chinese VZV strains reported in this study showed different molecular characteristics from those circulating in Europe, North America and Japan. The SNPs in ORF62 and ORF38 may be used to distinguish VZV wild-type strains and vaccine strain in clinical isolates in China.

ABSTRACT

BACKGROUND: The normal clinical index and the standard error of ultrasonic bone density in the population of normalchildren aged 6-12 years have not yet been established. OBJECTIVE: To investigate the status of ultrasonic bone density in the population of normal children aged 6 to 12 years old in Shenzhen City and establish a normal reference index of ultrasonic bone density of this population. DESIGN: Cross-sectional survey. SETTING: Ultrasound Department of People's Hospital in Longgang District of Shenzhen City. PARTICIPANTS: The volunteers for ultrasonic bone density detection were chosen froma kindergarten, an elementary school and a middle school between March 2002 and April2003. Excluded were those who had bone fracture and received medication of hormone and other drugs within 6months that affected bone metabolism, and menstrual history. Altogether we chose 697 normal children, 367 boys and 330 girls whose age ranged from 6 to 12 years old. METHODS: SAHARA ultrasonic bone density apparatus (the US) was used, and corrected according to the standard body model after the operation began.The error of precision was below 1% and the error of accuracy was 3%. Bone density of the left heel of the children was detected and body mass (kg) and height (cm) were also measured using the same height and weight scale. MAIN OUTCOME MEASURES: The correlation between bone densityof the heel and sex, age and body mass in the population of normal children aged 6 to 12 years. RESULTS: The normal reference values of bone density in the population from 6 to 12 years old were (0.445±0.166) g/cm2 in 6-year-old group,(0.509±0.151) g/cm2 in 7-year-old group, (0.510±0.133) g/cm2 in 8-year-old group, (0.519±0.132) g/cm2 in 9-year-old group, (0.520±0.153) g/cm2 in 10-year-old group, (0.53±0.175) g/cm2 in 11-year-old, and (0.545±0.206) g/cm2 in 12-year-old group. There were differences in bone density between boys and girls (P < 0.001), and after correction by body mass, the difference disappeared. Bone density of the boys and girls had increasing linear correlation with age (male r=0.722, P < 0.001; female r=0.785, P < 0.001), and had significant relationship with body mass (r=0.984, P < 0.001). CONCLUSION: In normal children aged 6 to 12 years old, bone density of the heel has no association with sex, but has increasing linear correlation with age and significant relationship with body mass.

ABSTRACT

Objective To understand the change of erythrocyte immunity and matondialdehyde (MDA) in perioperative patients with mammary cancer. Methods30 patients were divided randomly into two groups:group A(general anesthesia) and group B(epidural block). To estimate the activity of erythrocyte complement receptor I (CR- I ) and the lever of MDA, the venous blood were collected at the time points of before anesthesia, after anesthesia, during operation, and on the 1st, the 3rd, the 5th postoperative day. ResrltsCompared with before anesthesia, the RBCC3bRR, RBCCaR and RFER in group A were decreased statistically at the time points of after anesthesia, during operation, and on the 1st, the 3rd postoperative day(P <0.05 or P <0.01). Above mentioned index in group B were reduced significantly only during operation and on the 1st postoperative day. Between two groups, above mentioned index in group A were lower than those in group B(P<0.05).Compared with before anesthesia, RFIR and MDA in group A were higher at the time points of after anesthesia, during operation, and on the 1st, the 3rd postoperative day(P<0.05 or P<0.01).but above mentioned index in group B rose only on the 1st postoperative day.CR- I and MDA in two groups restored to the lever of be fore anesthesia on the 5th postoperative day. ConclusionThe perioperative immunological function of erythrocyte is obviously depressed by general anesthesia and epidural block, especially the former. This depression is relevant to the disorder of erythrocyte immunity regulating function and the lever of plasma MDA.
